I have enrolled in an online program offered by WomanUp Fitness. I recognize that it may involve strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular and conditioning training as well as other various activities.
I hereby affirm that I have checked with my doctor prior to beginning this specific program. I am in good physical condition and do not have any known disabilities or conditions that would prevent me from participating in this challenge. I hereby acknowledge that I am not pregnant at this time.
In consideration of this program, I hereby release WomanUp Fitness, it’s employees, and representatives from any liability now or in the future for conditions that I may obtain. These conditions may include but are not limited to: heart attacks, muscle strain, muscle pulls, muscle tears, broken bones, shin splints, knee injuries, foot injuries, or any other illness or soreness that I may incur, including death.
